Melissa McHam Green is the founder and principal of Crane Care Management. A Brooklyn native, she has spent her career helping New Yorkers. From her early years, throughout her education and in her professional life, Melissa has been committed to serving those in need. Providing care management to older adults is a natural extension of her lifelong mission. 

Melissa earned a BA in Sociology, and her Masters degree at Smith College School for Social Work in 2000. For the past twenty years she has been a licensed social work professional. Over the years Melissa worked as a Medical Social Worker at a local hospital in various units, including Neonatal Intensive Care, Oncology, Medical Surgical and outpatient Family Health Centers. In 2010, she was promoted to Director of the Social Work Department at a major Brooklyn hospital. As Director, Melissa managed the day-to-day operations of the department and a staff of twenty-two, including sixteen licensed Social Workers. She served on many committees, including Bio Ethics, Cancer, Palliative Care, Readmissions Prevention, Utilization Review, Policy and Procedures and Pastoral Care. Her experience navigating health care systems makes her an invaluable advocate for her clients. She knows how to efficiently move through the system to get what clients need.

            For the past several years Melissa has focused on honing her skills as a Geriatric Care Manager and joined the Aging Life Care Association.
